Anaphylaxis

Definition: systemic type 1 hyper sensitivity reaction, mediated by IgE. Characterised by laryngeal oedema, bronchospasm, hypotension.

Treatment

  1. remove offending stimulus
  2. ABC
  3. O2 + 0.9% saline
  4. 1 ml 1:1000 adrenaline im every 10 minutes (salbutamol if on β-blocker)
  5. chlorpheniramine 10 mg iv
  6. hydrocortisone 200 mg iv
If unresponsive, call the anaesthetist and give
  1. salbutamol nebulised in O2
  2. aminophylline
  3. ITU IPPV
After the attack,

  2. Epipen
